Our Hazard Identification (HAZID) Review, Plant Safety Review (Cold Eyes Safety Review) and Hazard and Operability (HAZOP) Review are structured methodologies aimed at identifying risks, assessing safety measures and recommending improvements to optimize facility safety and operability. Each review leverages detailed inputs like process descriptions, facility layouts and plant documentation. It brings together a multidisciplinary team and is facilitated by an experienced moderator for thorough and systematic evaluation.

The PLANTSERV® team also offers Safety Integrity Level (SIL) Assessments, Layer of Protection Analyses (LOPA) and Consequence Modelling with Quantitative Risk Assessment (QRA) Studies. These structured approaches provide a thorough evaluation of risks and safeguards to support compliance and effective risk management. Here also, our experts rely on detailed inputs and supporting documents, such as PFDs, P&IDs, equipment data and site-specific risk factors. They use advanced tools for accurate and actionable outcomes.

Consequence Modelling covers hazard scenarios such as cloud dispersion, explosion impacts and fire behavior (e.g., jet fires, BLEVE, fireballs). QRA, on the other hand, quantifies risks to individuals and groups, evaluates iso-risk contours and benchmarks tolerance levels against company, industry or regulatory standards. These analyses utilize tools like DNV PHAST™, Fire Dynamic Simulator (FDS), PyroSim, and DNV Safeti™ for precise modeling and decision support.

Root Cause Analyses (RCA), Reliability, Availability and Maintainability (RAM) Studies and Technical Qualification Reviews (TQR) are structured methodologies designed to investigate incidents, evaluate system performance and assess readiness for implementation. Each approach relies on detailed inputs, such as process diagrams, supporting documentation and expert facilitation, to deliver actionable recommendations for safety, reliability and efficiency improvements.

   •  Root Cause Analyses (RCA) investigate incidents by constructing a causal tree to identify root causes and define solutions to prevent recurrence. Facilitated by PLANTSERV RCA experts, this process incorporates timelines, workshops and additional analyses (e.g. material reviews or calculations). This aim is to support robust decision-making and enable effective preventive measures. PLANTSERV also includes training on RCA execution.

   • RAM Studies simulate equipment and system performance to analyze plant availability and identify causes of production losses. Using process data, maintenance schemes and reliability parameters, these studies examine alternative configurations with a view to optimizing reliability and minimizing downtime for enhanced operational efficiency.

   • Technical Qualification Reviews (TQR) evaluate the readiness of new systems, components or facilities for commercial use. The aim is to contribute to safety, environmental and economic compliance. This review identifies failure modes and mitigates risks through detailed assessments of technical viability, reliability and functionality. It empowers you to address potential risks to performance and compliance.
